If your request is to schedule a meeting with the Assistant Director, we will provide a link to do so on the next page. Please note: If a parent or outside party will be attending, you must inform us in advance and share the subject of the meeting. This ensures we are prepared. During the meeting, we will focus solely on the subject specified in your request. Any additional topics will require a new form submission and a separate appointment. If your request involves attendance concerns, please share the exact dates and hours you’d like us to review. For example: “9-5-25 should have 7 hours.” If you provide your current log or specific dates you believe should be included, we will go through them line by line for accuracy.
